How to Use Your Post-Discharge Meal Delivery Benefits

Blue Medicare Advantage understands that the transition of going home after a stay at a hospital, skilled nursing facility or rehabilitation facility can be overwhelming. It is also a critical period to assure you are receiving the proper care. One way we support members during this period is by providing a post discharge meal benefit.

Our plan has partnered with Mom's Meals to provide your meal benefit. After discharge from an inpatient stay you are eligible to receive up to 14 nutritious, precooked refrigerated meals delivered to your home.

Our team will work with you to coordinate delivery of meals from our vendor Mom's Meals.
